Foundation Medical Partners

Affiliated with Southern New Hampshire Medical Center, Foundation Medical Partners is the second largest multi-specialty group in New Hampshire. The Foundation includes over 180 providers and 47 practices, serving the greater Nashua community. Foundation Medical Partners usually refers primary care patients to this hospital and affiliated doctors. Many Foundation specialists also practice at other hospitals, including our tertiary partners – Lahey Clinic for cardiac care and Massachusetts General Hospital Cancer Center for cancer care, for example – with whom our physicians work cooperatively to coordinate patient care.
