How does my PCP know I am in the hospital?
Hospitalists communicate with your PCP when you are admitted to the hospital, during your hospitalization and upon discharge. They work closely with your doctor and specialists (if necessary).
What happens when I am discharged from the hospital?
When you are ready to leave the hospital, a hospitalist will communicate with your Primary Care Provider to discuss further treatment options, help arrange follow-up care, and prescribe necessary medications.
