How does my PCP know my child is the hospital?
Hospitalists communicate with your PCP when your child is admitted to the hospital, during the hospitalization, and upon discharge. They work closely with your child's doctor and specialists (if necessary).
What happens when my child is discharged from the hospital?
When your child is ready to leave the hospital, a hospitalist will communicate with your child's primary care physician to discuss further treatment options, help arrange follow-up care, and prescribe necessary medications.
