If you need a referral, please call our referral line at 672-6055.  This line is available 24-hours a day.  Please be aware any information left on this line after 4:00pm, on the weekend, or holidays will not be received until the next business day.  Please leave the following information:

 

• Patient Name
• Date of Birth
• If you were referred by our office
• A specific specialist's name and address
• Diagnosis or reason for visit
• Appointment date
• Name of your medical insurance company

Please allow 7 - 10 days for this referral to be processed.  If you need the referral faxed to the specialist's office due to time constraints, please provide the specialist's fax number. As always, you should check with your insurance company to determine what your benefits are as referrals do not guarantee payment.
