In order to ensure the best possible care, all new patients and returning patients should contact our Intake Coorindators at (603) 579-5136. Please leave a message on our voice mail line letting us know the phone number where you may be reached during the day.
Our staff will match you with the provider who best fits your needs, insurance coverage and clinical issues. Although we do offer some evening hours, there are specific days/times that new patient appointments are scheduled. Your first appointment will be scheduled through the front desk. Follow-up appointments will be made directly with your therapist. Follow-up appointments with a medication provider are scheduled through the front desk.
We do our best to ensure that the provider you are scheduled with is covered under your insurance plan. If there is a conflict with your coverage the Intake Coordinators will notify you and help you to make other arrangements, either with another provider here at Slatoff & Ward or at another facility in the area.
If you need to cancel a first time appointment, please call our Intake Coordinators at (603) 579- 5136 to let them know. We request a 48 hour notice for cancellations.
All first time appointments not cancelled within the requested time, or for which the patient "no shows," will incur a charge of $80. We will request that this charge be paid in full before another appointment is made.
Follow-up appointments that are not cancelled within the requested time frame will incur a charge of $50.
